What the OBBBA means for Medicaid program requirements

The "One Big Beautiful Bill Act" (OBBBA, H.R. 1) rewrites a large part of how state Medicaid programs are financed, who is eligible, and what states must do to keep federal funding. Some of it took effect the day it was signed. Some of it waits on federal rulemaking that will play out over the next several years.

We built the interactive guide below to sort through it. It covers the projected national impact, a requirement explorer that separates self-enacting provisions from those that still need federal action, deep dives on eligibility, financing, and benefit mandates, and a preparedness roadmap for state programs.

How to use it

Start with the Requirement Explorer and filter to "Self-Enacting". Those provisions are already in effect or have fixed dates, so they set the immediate work plan. Then switch to "Requires Action" to see what still depends on HHS guidance, which is where engagement with CMS matters most.

The roadmap at the bottom groups the work into the first 90 days, the first year, and the following one to three years. It is a starting point, not a substitute for your own fiscal modeling.
